Membership Application Submission Procedure

Purpose

To collect complete and accurate member information for application and facilitate efficient processing.

Scope

Applies to all individuals seeking membership in the Judo Club.

Roles & Responsibilities

  • Applicant: Completes the application form and submits it with required documents by the deadline.
  • Membership Coordinator: Receives and reviews applications, ensures completeness, and informs applicants of missing information.

Procedure Statement

  1. Download and read the Internal rules of the club:
  2. Fill up the membership application form from the club website or request a copy by email at cx@melliehajudoclub.org.
  3. Complete all sections of the form accurately and legibly.
  4. Ensure your date of birth, address, and contact information are up-to-date.
  5. Select your "Obi" level from the provided options.
  6. Provide the following required documents:
  7. Submit the completed application with all documents:
    • Online through the secure portal on the club website
    • In person at the club during classes hours
    • Via email to info@yukizunajudoclub.org

Exceptions

  • Incomplete applications without required documents will not be processed. Attendence to classes will be refused.
  • Applicants requiring assistance with the form can ask for guidance by email at info@yukizunajudoclub.org.
